What to include in a notice
A copyright complaint should include enough information to identify the protected work, the material or activity you believe is infringing, and where it can be found. Include your contact information so the request can be answered.
- Identify the copyrighted work or works at issue.
- Identify the material and provide a URL or other location where it can be found.
- Provide your name, address, telephone number and email address.
- Include a good-faith statement that the use is not authorized by the copyright owner, its agent or the law.
- Include a statement that the information in the notice is accurate and, under penalty of perjury, that you are authorized to act for the copyright owner.
- Sign the notice physically or electronically.
